Release Form for 2011-2012 YearClick HERE to download the release form for your teenager that will apply to all minor, local activities throughout the school year.  Fill this out and turn it in to the youth ministry department.  You can mail it to the church or drop it off in the metal box right outside of my office in the youth room.
